Parents and athletes need to fill out everything except the part the doctor fills out and have it notarized before they show up at the physical.
Also it must be completed by parents and a doctor and insurance must be paid before practicing.
Insurance prices are: Football- $76, other sports- $18. All insurance will be paid online through the student portal. Instructions are below.
Any and all senior high school student physical examinations for athletic purposes are to be recorded on the M-DCPS Athletic Physical Form/Eligibility Packet.
This physical form must be signed by the parents(s)/guardian(s) in the presence of a notary.
Please note that the parent(s)/guardian(s) and the student must sign in several places.
The updated version of the physical form is part of a packet of eligibility information and forms that need to be completed, notarized and returned to the athletic department before a student athlete may participate in the physical examinations administered by the M-DCPS Sports Medicine provider.
In order for a student to participate in any of the following Florida High School Athletic Association (FHSAA) and Greater Miami Athletic Conference (GMAC) interscholastic athletic activities: team practices, games, team try-outs, pre and post season conditioning, open gyms, open facilities, student athletic training programs, student team manager/statistician programs and cheerleading performed at any FHSAA sponsored sport, the Athletic Eligibility Packet must be returned to the athletic office and the student must participate in the M-DCPS mandatory athletic insurance program for 2022-2023.
