Items needed for Registration

If you are transferring from another Miami-Dade County Public School, please provide the following;

  1. Proof of address in parent/legal guardian’s name: FPL bill, MDWAS, Food Stamp letter, or SSI benefits letter
  2. Current Driver’s License or State ID with picture

If you are transferring from another county, state, or country,  please provide the following;

  1. Items 1 & 2 from above
  2. Birth Certificate
  3. Immunization and Physical (forms #DH680 and DH3040)
  4. Official Transcript/Last report card

If you are not the parent/guardian of the student and you have temporary guardianship, you must present custody papers.

Documentos necesarios para matricularse

Si viene de una escuela publica del condado de Miami-Dade, por favor entregue el siguiente;

  1. Prueba de domicilio con el nombre del padre o tutor (guardian) como la factura de la luz/agua, papeles oficiales del gobierno, etc.
  2. Identificacion con su direccion (licencia de cunducir, carnet de identificacion del trabajo con foto, pasaporte, etc.)

Si viene de una escuela privada, de otro condado, estado o pais, por favor entregue lo siguiente;

  1. Vea #1 arriba
  2. Vea #2 arriba
  3. Acta de nacimiento o pasaporte
  4. Vacunas y examen fisico (formularios #DH680 y DH3040)
  5. Notas/calificaciones de escuelas anteriores (de 9, 10, 11 y/o 12 grado)

Si usted no es el padre/madre del alumno, necesitamos documentos oficiales de la corte o el gobierno dandole custodia del menor de edad.
