Community Health of South Florida Inc. (CHI)

South Dade Senior High health team consist of a Nurse Practitioner or Physician Assistant, Social Worker and Health Aide

The Children’s Trust of Miami-Dade County, The Florida Health Department and Miami-Dade County Public Schools have joined together with Community Health of  South Florida Inc. (CHI) to provide medical services at your child’s school through a program called Health Connect in our Schools.

 

South Dade Senior High health team consist of a Nurse Practitioner or Physician Assistant, Social Worker and Health Aide (Nursing Assistant or Medical Assistant) who are ready to meet your child’s medical needs Monday through Friday.  We offer school physicals and vaccinations by appointment only.

 

Do you require medical services on additional children who do not attend the same school?   Make an appointment our health team can provide additional services to meet all of your children’s medical needs.

 

Does your child have health insurance?  If not another option we offer is assistance in obtaining health insurance for your child through the following program Health Connect in our Community. A Health Navigator can provide you with additional information on the above services call our health suite for more information.
